On commission from ConocoPhillips, Mintra has visualised several different unwanted incidents. Among other things, these e-learning modules are used at safety meetings to address incidents and learn from them.

The e-learning modules are developed from real Unwanted Incident Reports, and contain the following three elements:

• Presentation of the incident.
• Why did it happen?
• How can we avoid similar incidents in the future?
